Free flat head photo assessment form

If you’re worried your baby might have flat head syndrome (plagiocephaly) we’re here to help.

Use our flat head photo review form for free orthotic guidance on possible next steps.

Send us photos of your baby’s head and one of our orthotists will review them and let you know whether the head shape appears consistent with common features of positional plagiocephaly or brachycephaly.

Photos alone cannot confirm a diagnosis, determine severity in every case or rule out other causes of head shape difference, but they can be a helpful first step.

If we think an in-person orthotic assessment may be helpful, we’ll explain what happens next and how to book.

How it works

1. Take clear photos of your baby’s head from different angles:

2. Attach the photos to the form below.

Please ensure each photo is under 2MB (you might need to make them smaller if uploading directly from your phone or camera).

3. Complete the form with your details, hit submit, and one of our orthotists will get back to you.

Book a free initial orthotic consultation

We offer free orthotic consultations for babies aged three months and older.

During this appointment, we take a 3D scan and measurements of your baby’s head for orthotic assessment purposes.

We use this information to discuss whether repositioning may still be appropriate or whether helmet therapy may be an orthotic treatment option.

We do not provide medical diagnosis or rule out other conditions. If medical assessment is needed, we recommend speaking with your GP, paediatrician or another appropriate medical professional.
